Pamela Schasel, LMSW, CMC

Pamela Schasel, LMSW, CMC, joined Limestone Primary Care Physicians, LLP in August of 2019 bringing over 28 years of social work experience. 

She received her Bachelor’s degree with a major in human services from Medaille College in Buffalo and her Master’s degree in Social Work with a concentration in mental health and aging from the University of New York at Buffalo. Pam is a Licensed Social Worker in New York State and a national Certified Care Manager.

Pam and her husband have five children and two granddaughters between them. She enjoys spending summers at Lake Chautauqua with her family and friends. 

Pam is working in our practice and will be initiating a Behavioral Health Care Management program to improve the overall health of our patients. She will be working with our providers and a psychiatric consultant to identify and address patients’ mental and behavioral health needs and developing strategies to meet those needs. This will include management of illnesses such as anxiety, depression, chronic pain, dementia, alcohol and substance abuse. Pam will facilitate engagement in linking patients requiring a higher level of care to the appropriate specialists such as mental health counselors and psychiatrists. She will provide support and resources to patients and their families. The program will engage patients through telephonic contact as well as in person meetings at our practice.
