Giving Doctors Extra Support to Help You Get Better Care

Limestone Primary Care Physicians is participating in CPC+, the nation’s largest-ever program to improve primary care.
Through CPC+, our insurers will give our practice additional resources to help us better manage your care.

To help us take better care of you, Medicare will start sharing some of your personal health information with us.  This information will help provide us a more complete picture of your health so we’re better able to coordinate your care.

If you want to stop Medicare from sharing this information, please let us know as soon as possible so we can notify the CPC+ team.

Your Medicare benefits aren’t changing.  You still have the right to use or visit any doctor or hospital that accepts Medicare, at any time.
Your doctor may continue to recommend that you see particular doctors for your specific health needs, but it’s always your choice which doctors you use or hospital you visit.

Preventive Health for 2015-2016


We are pleased to have a “Patient Portal” available for our patients and their family if the patient gives their family member(s) access.

  • The portal allows you to have real-time access to your medications, labs and upcoming appointments.
  • You are able to send NON-URGENT messages to your provider avoiding phone calls.
  • Patients can request refills through the portal as well.
  • You are able to update your demographics, insurance information and health history through the portal.

The sign up process requires an activation code which must be obtained from our office. If you would like more information, please contact us or ask your nurse at the time of your visit.

Limestone Primary Care Patient/Family Advisory Board

As part of our continued effort to provide the best medical care to our patients, we have formed a Patient/Family Advisory Board. The Board consists of Limestone staff including a physician, Dr. Scrivani; one of our PA’s, Katie Braisted; Dr. Shafik’s nurse, Elaine; our front office supervisor, Jennifer and our Office Manager, Cheryl Reynard. We were lucky to have 14 patients volunteer to be on the board. Our Mission Statement is as follows: Limestone Primary Care Physicians Patient and Family Advisory Board will be dedicated to improving the overall healthcare experience by providing a voice to all who utilize our services. Our first meeting took place on April 27, 2016. We had a great discussion regarding:

  • The most effective way to provide patient education to our patients
  • The “Team” concept and what it means for the patient, family and our staff.

The group had a lot to share and many good ideas. We are meeting again in June and will provide an update on what we are trying to accomplish and our progress. If you are interested in joining the Board, we would be happy to have you. Call the office to inquire about the next meeting.

High Deductible Health Plans

Due to the increased number of patients with “high-deductible health insurance plans”, we will be collecting a $50 minimum payment toward your deductible. (Physical exams require no payment) This payment is expected at the time of your visit. We encourage patients to review their insurance policies and be aware of the coverage and costs associated with it.

Urgent Care Centers

The Providers at Limestone Primary Care feel that Urgent Care Centers provide a service for our patients as an alternative to utilizing an emergency room.On the other hand, we would like to discourage our patients from utilizing these centers when we can offer extended care hours or medical advice for our patients during the day or on weekends. We have extended our hours to start earlier in the morning and provide later evening hours. We are also open Saturday mornings. We are able to accommodate same day appointments for our patients and the “on call physicians” are able to provide after hour advice for urgent situations. We feel that as your Primary Physicians, we can offer more comprehensive and knowledgeable treatment for our patients due to the fact that we have access to your complete medical record.


A one on one meeting with a Registered Dietitian will help you achieve your nutrition related goals faster and improve your health. During your sessions you will get a customized plan to meet your specific needs and goals. Through regular follow up sessions you will have a chance to see your results, get more help in the areas where you struggle and have the support you need to stay on a healthy path.

WE are pleased to have two Dieticians on site for your convenience. Talk to your provider about making an appointment with them.

Allyson Odachowski, RDN, CDN is in our office on Thursdays from 9am-1pm. You can call our office to schedule your appointment with Allyson. Below is a link to her website where you can find her office information as well. Allyson accepts most insurances and is happy to make sure your plan covers her services.

Stefani Tubin, RDN, CDN is in our office on the 3rd Monday of every month from 11am to 2pm. This is a covered service for patients who have Blue Cross Blue Shield of WNY. Appointments can be scheduled through our office.


Diagnostic X-ray performs ultrasounds in our office on Monday and Friday mornings from 8am to 12pm. Provider orders and appointments are required.
