Welcome To Limestone Primary Care Physicians

Thank you for choosing us to serve your family's healthcare needs. Our practice consists of three board-certified Physicians, a Family Nurse Practitioner and a Physician Assistant. We are conveniently located in Buffalo, NY in the village of Williamsville. As primary care doctors, we offer comprehensive health care to patients of all ages.

New Patients

1.  Schedule a New Patient visit with your Physician.  Your first visit will include a physical exam.

2.  Print and complete the Patient Registration Form.

3.  Print and complete the Health History Questionnaire.

4.  Print and complete the HIPPA form.

5.  Prepare for your visit.  In addition to the above forms, please bring the following:

  • Insurance card and copay
  • List of current medications
  • Vaccine record from pediatrician or previous physician

6.  Plan on arriving at least 10 minutes prior to your appointment time.
